Preventive and Interceptive Orthodontics: Management of Anterior Open Bite and Premature Dental Loss. Treatment focus report

Background: The current approach to treatment in children and adolescents implies a broader and holistic look due to its comprehensive and multidisciplinary management. Multiple factors must be considered in the clinical evaluation and pediatric dental treatment, encompassing both the management of the cariogenic risk approach and craniofacial growth and development. In this clinical case, there is a functional and aesthetic compromise; Therefore, together with the management of cariogenic risk factors and rehabilitation treatment, a preventive and interceptive orthodontic treatment was implemented.
Clinical Presentation: 9-year-old school patient undergoing the first transitional period of mixed dentition, Marked caries activity due to deep carious lesions, Angle Class I Malocclusion with dentoalveolar anterior open bite due to non-nutritive sucking habit, finger and tongue interposition at rest and swallowing. The preventive and interceptive orthodontic treatment involved the management of the space with a fixed lingual arch maintainer due to the premature loss of the second lower primary molar and a maxillary expander with a palatal grid, to re-educate the tongue to avoid lingual interposition and recover loss of transverse growth of the maxilla.
Clinical Relevance: The preventive and interceptive orthodontic treatment administered by the general dentist implies diagnostic skills and proper clinical judgment. For the success of this treatment, it is necessary to consider that the patient is in Neutroclusion; which considerably improves the prognosis. This type of treatment is relatively fast to solve, and a key factor is periodic monitoring since any complication must be referred to Orthodontics.
Conclusion: The timely treatment and preventive management of the loss of space and the interception treatment of dentoalveolar anterior open bite avoids a subsequent secondary skeletal alteration and is a treatment to be performed by the general dentist.
